Abstract:

India continues to lack sufficient evidence which proves the connection between perinatal asphyxia and autism spectrum disorder (ASD) in Indian children despite having a serious problem with the condition. The researchers conducted their study at an Indian homeopathic medical institution to find out whether perinatal hypoxia causes ASD while testing the effects of Hyperbaric Oxygen Therapy (HBOT) on patients who have perinatal hypoxia and ASD. The tertiary neurodevelopmental unit accepted 428 children between 2 and 10 years of age who had DSM-5 ASD diagnosis from October 2021 until September 2024. The research paper provides both descriptive data and hypothetical data which researchers have verified through existing Indian and international studies. The Indian Scale for Assessment of Autism (ISAA) served as the tool for measuring ASD severity. The researchers conducted a nested randomized controlled trial (RCT) which involved 64 children who had perinatal hypoxic histories and moderate-to-severe ASD. The study also incorporated contextual evidence from CCRH and NHRIMH Kottayam. The study found that 157 children from a total of 428 showed evidence of perinatal hypoxic events which amounted to 36.7 percent of the total group. The subgroup showed an earlier onset of symptoms which began at 18.4 months compared to 23.1 months with a statistically significant difference of p<0.001. The group with hypoxic histories showed higher IL-6 serum levels which reached 18.4 pg/mL compared to the control group who had 11.7 pg/mL with a difference that reached statistical significance at p<0.001. The nested RCT results showed that HBOT treatment led to better ISAA score reductions which reached −18.3 compared to the control group which achieved −9.4 with a significant result of p<0.001. The adverse events showed low severity and resolved without treatment. The research findings show that Indian children develop a more severe ASD phenotype because of perinatal hypoxia which creates a separate disorder pattern. The biologically defined subgroup showed significant behavioural and neuroinflammatory improvements after HBOT treatment. The assessment of ASD should include perinatal history-taking as a standard practice while further multicentre RCTs need to investigate HBOT treatment for perinatal hypoxia-induced ASD.
